Initial evaluations - $150.00
Individual follow-up sessions - $120.00

Please note that insurance companies typically have two types of cost-sharing arrangements after the policyholder meets their deductible. Co-insurance and Co-pays. Co-pays are a fixed rate and Co-insurance is a percentage. Ask the insurance what your cost will be.
